Chronic Disease Program Offered Locally |

Morrison Parish Nurses, CGH Medical Center, Sterling and University of Illinois Extension, Whiteside County is pleased to offer “Live Well, Be Well,” an internationally recognized chronic disease self-management program developed by Stanford University’s Patient Education Research Center. “Live Well, Be Well offers people who experience the challenges of living with one or more chronic health conditions information to develop confidence, skills, and knowledge to more effectively manage the condition and regain control of their lives. Classes are highly participative, taught by trained and certified instructors, and build upon the successful outcomes of each individual. The program is scheduled to being July 1st, and will be offered once a week on Tuesday nights for seven weeks from 6:30-9 p.m. at the Morrison United Methodist Church. Registration is limited and applications are requested by June 27th. The cost for the program is $25, which includes all session, books, and related materials.
