This is applicable to anyone 18 years of age and over.
For those individuals exhibiting COVID-19 symptoms, our Family Care Clinic is providing testing by appointment only. Please call 815-772-5511 to schedule an appointment. Stay in your car and again call 815-772-5511 to let the Receptionist know you have arrived. Wait in your car until you are called to be roomed. Your test results will be provided within 24 hours. Please do not call the clinic or hospital; someone will call you directly with your results.
