One of our goals at Hospice of the Rock River Valley, a United Way Agency, is to promote the integration of comprehensive, high-quality, seamless delivery of end-of-life care for individuals, families, and caregivers in our community.
We invite you to participate in a Community Awareness Survey and would appreciate your help in identifying concerns about end-of-life planning and care. Your responses are confidential and will make a real difference.
The survey can be found on the homepage of our website, www.hospicerockriver.org, and will take approximately five minutes to complete. If you would prefer that a survey be mailed to you, please call the office at 815-288-3673. The survey will remain open until Saturday, May 31, 2014.
